Diana, Thanks for the great post. I tried like heck to find more info about each of the practice improvement activities on the CMS website–as best I can tell the only guidance they post are these blurbs that are only a few paragraphs. Do you know of any other guidance from CMS in terms of exactly what they are looking for and in particular, what are they looking for in terms of documentation? They really don’t seem to be giving us any help in terms of satisfying a potential auditor.
